The process

  1. 1

    CHE meets with your facility to discuss the unique needs of your population and design a program of care that fits your patients, staff, and operations.

  2. 2

    We train facility staff to identify residents who may benefit from psychological services, including depression, anxiety, psychosis, adjustment concerns, hoarding, behavioral challenges, grief, and the psychiatric symptoms of dementia.

  3. 3

    When a referral is identified, a physician order is placed and the patient is connected with the appropriate CHE specialist.

  4. 4

    After an initial evaluation, the provider recommends a treatment plan that aligns with the facility's Care Plan.

  5. 5

    If clinically warranted, the CHE provider delivers individual, group, family, or medication-management treatment under that individualized plan.

  6. 6

    The provider documents and communicates care planning to facility staff through charting, discussions, and care meetings, in line with regulatory guidelines.
